Tips in filling-out Panama Application Form

Liaison Officers must double-check application forms especially if DONE by OJT's and Utilities:

Panama applications are evaluated from left-to-right, top-to-bottom. The first (1st) thing an evaluator sees, is the picture and the application form itself. Evaluators check the NAME (e.g. First Name, Middle Name, Surname) Date of Birth and Passport Number. These data must coincide with the copy of passport attached. It is more important that you attach the passport next to the application form. Most computer generated application forms BEARS an OLD passport number. Check your application forms before submitting.

Inline with the Passport Number is the Certificate Number and Expiration Date (in filling-out the application form) . These information must also coincide with the COC attached. This is checked together with the letter request if the position requested matches the certificate of competency or the certificate of assessment. Letter request must be signed by an authorized signatory of the company.

Basic safety course must be attached for ratings, and must not be more than 5 years from date the certificate was issued.

Lastly, the Medical Certificate. Must be original, in panama medical form, and signed by the physician with stamp, telephone number and address of the clinic.

To evaluate your application faster, you may arrange your documents with the following order:

Officers:
1.) 8 pcs. 3 x 3 cm Photos
2.) Panama Application Form
3.) Copy of Passport
4.) Letter Request (Stating Position Requested and Vessel's Particulars)
5.) Certificate of Competency
6.) Certificate of Endorsement
7.) Medical Certificate (Original and in Panama Medical Form)

Ratings: (Watchkeeping: AB/OILER/BOSUN/MOTORMAN/G.P.)
1.) 8 pcs. 3 x 3 cm Photos
2.) Panama Application Form
3.) Copy of Passport
4.) Letter Request (Stating Position Requested and Vessel's Particulars)
5.) Certificate of Competency (TESDA)
6.) Basic Safety Course
7.) Medical Certificate (Original and in Panama Medical Form)
8.) Seaman's Book with Sea Services

Ratings: (Non-Watchkeeping)
1.) 8 pcs. 3 x 3 cm Photos
2.) Panama Application Form
3.) Copy of Passport
4.) Letter Request (Stating Position Requested and Vessel's Particulars)
5.) Certificate of Assessment (from Accredited Training Center, endorsed)
6.) Basic Safety Course
7.) Medical Certificate (Original and in Panama Medical Form)
8.) Seaman's Book with Sea Services

2. You can use your Company PESO CHECK. (not necessarily CASHIER'S / MANAGER'S CHECK).

3. Make all your checks payable to PANAMA CONSULATE.

4. Panama Consulate NO LONGER accept CASH Payments for security reasons.
